We recommend and encourage each client to review the following terms prior to booking and prior to each appointment.

AZURE SWAN REIKI & BODYWORK, LLC TERMS & CONDITIONS

APPOINTMENT BOOKINGS & CANCELLATIONS

Services and sessions may be booked online or by telephone. A valid credit or debit card is required; the card will not be charged unless there is a missed appointment.

Services may be booked up until 12 hours prior to the selected time slot.

Appointment reschedules and/or appointment cancellations must occur within 48 hours prior to the scheduled appointment. Cancellations or reschedules occurring less than 48 hours prior to the appointment time will be charged a $25.00 fee.*

No-call/no-show clients will be charged the full session/service rate plus 18% gratuity for missed appointments.*

Additional charges may be incurred for add-on services.

Minors under 18 years of age must have their parent or legal guardian book for them.

Minors under 18 years of age must have written consent of their parent or legal guardian in order to participate in and/or receive sessions and/or services.

DISCOUNTS, PROMOTIONS & OTHER SPECIAL PRICING

Promotions, discounts, and other special pricing cannot be combined unless otherwise stated.

One promotional/discount/special pricing code per person per visit.

Promotional rates, discounts, and other special pricing cannot be transferred unless previously agreed to by Azure Swan Reiki & Bodywork, LLC and its Management and/or Members.

Promotional rates, discounts, and other special pricing are valid for 30 days unless otherwise stated or agreed to by Azure Swan Reiki & Bodywork, LLC and its Management and/or Members.

Services purchased at a promotional or discount rate must be redeemed within 90 days or will forfeit the special pricing rate, unless previously agreed to by Azure Swan Reiki & Bodywork, LLC and its Management and/or Members.

Retail goods purchased at a promotional rate are eligible for a full refund at the promotional rate if returned within 30 days from the date of purchase.

Minors under 18 years of age cannot claim, redeem, or use promotional rates, discounts, or other special pricing without the written consent of their parent or legal guardian.

CONTESTS & GIVEAWAYS

Contests and/or giveaways are held at the discretion of Azure Swan Reiki & Bodywork, LLC and its Management and/or Members.

Contests and/or giveaways may be closed, cancelled, or otherwise terminated by Azure Swan Reiki & Bodywork, LLC and its Management and/or Members for any reason and at any time.

Minors under 18 years of age cannot participate in contests and/or giveaways without the verbal or written consent of their parent or legal guardian.

GIFT CERTIFICATES

Gift certificates are non-refundable.

Gift certificates will only remain valid and must be used within 120 days from date of purchase, unless previously agreed to by Azure Swan Reiki & Bodywork, LLC and its Management and/or Members.

Gift certificates may be utilized for coverage of session/service costs, gratuities, and/or retail purchase costs, provided that the certificate amount is sufficient to cover the total transaction cost. If the certificate amount is insufficient to cover the total transaction cost, a valid credit or debit card may be used to cover the difference in cost.

CONSUMABLE & NON-CONSUMABLE RETAIL GOODS

Retail goods such as soap dishes, candle trays, eye masks, and other non-consumable goods are eligible for a full refund if returned unused within 30 days from the date of purchase.

Retail goods such as lotions, oils, candles, and other consumable goods are eligible for one (1) exchange due to allergy, sensitivity, and/or other intolerance of the product, and only if no less than two-thirds (2/3) of the original product remains. This exchange must occur within 30 days of the date of purchase. After the one (1) exchange, no exchanges will be permitted, nor will a refund be issued.

DISTANCE & REMOTE APPOINTMENT CHECK-IN

Azure Swan Reiki & Bodywork, LLC and its Management and/or Members request that distance and/or remote clients who are participating in phone and/or video call sessions please dial in or otherwise connect no more than five (5) minutes prior to their scheduled appointment time.

Clients with phone and/or video call sessions who have not dialed in or otherwise connected within 10 minutes after their scheduled appointment time will be considered as no-call/no-show clients and will be charged the full session/service rate plus 18% gratuity.*

IN-PERSON APPOINTMENT CHECK-IN

Azure Swan Reiki & Bodywork, LLC and its Management and/or Members asks that in-person clients please arrive 10-15 minutes prior to their scheduled appointment time. Late arrivals may be accommodated at the discretion of Azure Swan Reiki & Bodywork, LLC and its Management and/or Members; however, the length of the session and/or service will be adjusted so as not to interrupt the scheduled appointments of other clients. The full session and/or service rate will apply to late-shows; in cases of sessions and/or services purchased at discount, promotional, or special pricing rates, the full adjusted rate will apply.

In-person clients are welcome to utilize the common relaxation areas both pre- and post-appointment, provided that they do not arrive more than 30 minutes prior to nor remain more than 30 minutes after their scheduled appointment time.

GRATUITY

Gratuity is not included in the session and/or service rate.

Gratuity is at the discretion of the client, except in cases of no-call/no-show. No-call/no-show clients will be charged the full session and/or service rate plus an 18% gratuity.

Gratuities may be made directly to the session/service provider, or upon check-out at the front desk. Gratuities may be paid using cash or a valid credit or debit card.

ETIQUETTE

As a courtesy to other clients and to preserve the relaxing atmosphere of the space, Azure Swan Reiki & Bodywork, LLC and its Management and/or Members requests that clients please turn off their mobile phones at all times within all areas of the space.

All clients are asked to respect other clients by keeping noise to an agreeable level throughout the space.

Azure Swan Reiki & Bodywork, LLC and its Management and/or Members reserve the right to ask any client and/or group of clients to leave the space if they are disturbing other clients.

The entire space of Azure Swan Reiki & Bodywork, LLC is non-smoking.

PERSONAL ITEMS & VALUABLES

Neither Azure Swan Reiki & Bodywork, LLC nor its Management and/or Members accept any responsibility for the safety of a client’s personal items, money, and/or valuables of any kind that are brought into the space.

Neither Azure Swan Reiki & Bodywork, LLC nor its Management and/or Members will be liable for any loss of and/or damage to a client’s personal items, money, and/or valuables or any kind that are brought into the space.

COMMUNICATIONS

All communications made to Azure Swan Reiki & Bodywork, LLC and its Management and/or Members, including but not limited to questions, feedback, comments, and/or suggestions shall be and remain the property of Azure Swan Reiki & Bodywork, LLC and its Management and/or Members. Therefore, Azure Swan Reiki & Bodywork, LLC and its Management and/or Members will be entitled to use, reproduce, disclose, publish, distribute, and otherwise utilize any material a client submits for any purpose whatsoever, without restriction and without compensation to the client.

WEBSITE USAGE

Azure Swan Reiki & Bodywork, LLC utilizes a website that may provide you with the ability to use usernames, passwords, or other codes or devices to gain access to restricted portions of the website. The content contained in such restricted areas is confidential and is provided for your personal use only.

Azure Swan Reiki & Bodywork, LLC and its Management and/or Members reserve the right to prohibit the use of such access methods on your behalf by third parties where Azure Swan Reiki & Bodywork, LLC and its Management and/or Members determine that such use interferes with the website’s operation(s) and/or results in commercial benefits to other entities at detriment to Azure Swan Reiki & Bodywork, LLC and its Management and/or Members.

You agree that you will not use any device, software, or other instrumentality to interfere or attempt to interfere with the proper working of the website.

The website and all content available on this website are provided without warranties of any kind, either express or implied.
